Falling Slowly-Intermediate Errors

There are two errors that I can see in this sheet music. A mislabeled number, and the chords are not labeled the same when they same notes are being played. Also, I was wondering if someone could explain the nomenclature behind the inverted triads.. for example C/Something. What does the following /Note exactly mean?
